What is Case Management?

Case Management is..

We have been providing case management services since 2015, to people who have experienced brain injury, spinal injury, cerebral palsy and multiple trauma. We are an Irish based company with a wealth of experience in nursing, special needs and legal services.

We commence all of our relationships by firstly meeting the client and their family and explaining the role of the Case Manager and the advantages of having this service. We provide a personal service country wide and are available on the phone for support in between monthly meetings once the service is established.

Case Management is a collaborative process, that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s needs including health and wellbeing, education and/or occupational needs, using communication and available resources to promote quality, cost effective and safe outcomes.

We organise and manage therapy support in and outside the home.

Assessments & Reports

Immediate Needs Assessment: This is the assessment of the injured person’s initial needs in regard to care, support, medical, therapy, equipment, accommodation, vocational, educational, leisure, holidays, vehicle assess and transport.

Periodic Payment Orders: Preparing Case Management Reports and Costings for mediation hearings and High Court Hearings.
